Club/Organization Membership Renewals

1.  You must have at least one swim coach with all of their certifications currently up to date.

2.  Complete all of the 2012 club/organization application form:
        Download 2012 club/organization application form.

3.  Fill out the 2012 Registration Summary form (part A).
      Download 2012 Registration Summary form.  

4.  Send the club/organization application form, the registration summary form and fee to:
